HealthDrive is seeking a full-time Chronic Care Management Consent Coordinator to join our team in our corporate office in Framingham, MA.

 

The Chronic Care Management (CCM) Consent Coordinator is responsible for contacting health care proxies and/or the overall responsible person for a long-term care resident to obtain consent for participation in the CCM program.  This role involves clear communication, empathy, and a thorough understanding of the CCM program to effectively explain its benefits and requirements.  The Consent Coordinator will ensure that all necessary documentation is completed and stored appropriately.

 

We are conveniently located off Route 9 in Framingham, MA, close to Routes 90 and 495 in a spacious modern office with a workout center available right in the building!

Responsibilities

  • Make outbound calls to those responsible for a long-term care resident who is eligible for the CCM program to explain the benefits, answer questions, and obtain verbal or written consent for enrollment.
  • Clearly and concisely explain the details of the CCM program, including services provided, potential costs, and the benefits of participation.
  • Accurately document all interactions, consents obtained, and any follow-up required in a spreadsheet and EMR format.
  • Provide regular updates to management.
  • Regular reporting on the number of consents obtained, refusals, and trends.
  • Occasional weekend or off hours work required, less than 20%

Qualifications

  • Strong communication and interpersonal skills.
  • Ability to explain complex medical programs in simple terms.
  • Proficiency in using EHR systems, Excel spreadsheets, Microsoft Teams, and other relevant software. 
  • Strong organizational skills and attention to detail.
  • Ability to handle sensitive patient information with confidentiality and professionalism.  
  • Minimum of a High school diploma or GED required.  Associate’s or Bachelor’s degree in a healthcare-related field preferred, or 1-3 years of related experience.
  • Ability to sit for long periods of time.
  • Ability to use office equipment such as computer, phone, fax and copier.
  • Ability to lift up to 10 lbs. occasionally for duties related to office work.
